Super Survey

Super Survey lets you create surveys for your site and have your visitors fill them out. Add some interactivity to your site with surveys for fun or for research! You can create an unlimited number of surveys, and each one can have an unlimited number of questions.



For each question, you specify how the answers will be presented to visitors: in a select list, as radio buttons, as check boxes, or as a text input field. Each question can have up to 10 predefined answer choices. When a user fills out a survey, their responses are tabulated and added to the database.



Super Survey's administrative interface lets you easily add surveys, disable surveys so that no one can take them anymore, and view the results of any survey. Optionally, you can also allow your visitors to view survey results as well. Check out the demo to see Super Survey in action!
